Enrollment Fee: $150 per person, $250 for a couple (same household), $300 for a family (up to 2 parents and all dependents in the same household under 26).

Price does not include lab fees, imaging fees or certain in office procedures.

**The provision of Direct Primary Care Medical Services provided by Practice is not subject to regulation by the Texas Department of Insurance; is NOT a contract for health or accident insurance; and does not provide healthcare or accident coverage. It will only cover the services as described in the DPC Program Agreement. Patient should obtain or keep such health insurance policy(s) or plans that will cover Patient for general healthcare costs.
